    Use the link below to get a PDF file provided by the Florida Division of Elections.  You can fill out the form on your screen, print and mail the form to the address provided.  If you do not receive your Voter I.D. card within three weeks, you must contact your Clerk to find out what has happened to your application.
    To better ensure you get your Voter I.D. card, fill out the form, print it out and take the form to your Township Clerk to register.  Have him or her provide you with a receipt for the form and keeping checking to make sure you get the Voter I.D. card.  In the event you do not get the Voter I.D. card by the time the election rolls around you should be still be able to vote by providing the receipt as proof that you have registered.
